DHCF: Prior Authorization (PA) Requirement Removal for Nexplanon Etonogestrel implant 68 mg radiopaque (Procedure Code J7307)

The purpose of this transmittal is to notify the Medicaid providers that the prior authorization (PA) requirement for Nexplanon Etonogestrel implant 68 mg radiopaque procedure code J7307 will be removed effective April 1, 2018.
