Alignment of Plan of Care Dates with Medicaid Eligibility Certification Dates

The purpose of this transmittal is to notify Home Health Agencies (HHAs) of their responsibility to align Plan of Care (POC) dates to the Medicaid eligibility certification period for individuals enrolled in the Home and Community-Based Services Waiver for the Elderly and Persons with Physical Disabilities (EPD).
